Indiana health officials are asking Hoosiers in rural communities to share their experiences and concerns through a new survey now open through Friday, October 3.

The Indiana Department of Health says the feedback will be used in the state’s application for the federal Rural Health Transformation Program, which was created to strengthen healthcare in small towns, farming areas, and other rural regions.

The “Rural Community Health Priorities Survey” asks residents and workers in rural areas to rate their access to healthcare and prevention programs, identify barriers such as transportation, provider shortages, or broadband access, and suggest solutions to chronic disease management. Questions also cover mental health services, specialty care, and which types of health professionals are most needed in their counties.

The survey also explores ideas like mobile clinics, school-based health centers, home visits, and new uses of technology such as telehealth or remote monitoring. Respondents may share additional comments on what would help improve healthcare delivery in their communities.

Officials say the results will directly shape Indiana’s grant application and future planning for rural health improvements.
